not able to add new website in control panel

successfully installed SEO but unable to add website URL in "new website". after entering all details, when i clicked submit button, screen got freezed and only I can see is status bar. how to fix this problem

installed SEOPanel. but while accessing http://www.domain.com/seo I am getting "404-File or directory not found." error. I checked twice there file. Installed this webapp successfully but even thn I am getting this error. please help to fix this.
